If you are a NEW coach, or one who does not have a check mark in the CORI Verified box on the Certificates tab in your MYSA SportsConnect account, there is a one-time validation of identification required by the Commonwealth of MA. This can be done after applying for your CORI and does NOT have to wait until your CORI is processed.
- Login to your existing Sports Connect Account
- Click on the Print Documents link under your picture in your Sports Connect account then select Receipts & Forms on the line that says ADULT/CORI Registration to print out or download a copy of the two page CORI Acknowledgment Form.
- Your information on the CORI Acknowledgment Forms must be verified. There are two ways to do that (also see this video from MYSA that clearly demonstrates the CORI verification process for new coaches):
- Bring your CORI Acknowledgment Form and your driver’s license or passport to the Coaches’ Meeting in August and the Registrar will verify your identity. If you cannot attend the August meeting, email the Registrar at registrar@lexingtonunited.org and schedule a time to meet.
- Take the Form and your driver’s license to a Notary Public to get your information NOTARIZED. You will need to sign the form in front of the Notary. Lexington Town Offices offer free notary public services.
- Then, send a PDF or picture of the notarized form via email to: registrar@lexingtonunited.org
- DO NOT upload the notarized Acknowledgment form into the system. You send it to the Registrar and they will upload a separate CORI Verified form. Once that is done, you will see a check mark in this box. This only has to be done ONCE.
